I, the undersigned, am the parent or legal guardian of the camper named above and give my permission for their full participation in BrainEducators Summer Camp. I understand that participation in camp activities may involve physical activity and carry a risk of injury.
I acknowledge and agree that BrainEducators, its staff, volunteers, contractors, and affiliates are not liable for any injuries, accidents, or other mishaps that may occur while participating in the camp, whether on-site or off-site.
I agree to indemnify and hold harmless BrainEducators from any claims, liabilities, or expenses arising out of or related to my child’s participation in the camp.
In the event of an emergency, I authorize the staff of BrainEducators to secure emergency medical treatment for my child, and I accept full responsibility for any resulting expenses.
Behavior Expectations
I understand that BrainEducators reserves the right to remove a camper from the program if their behavior is disruptive, unsafe, or disrespectful to others. No refund will be issued in such cases.
